There are three versions of the inspection report 31.18 to enable compatibility with other software platforms. You may have to ‘enable macros’ before you begin in order to activate the software. Please see guidance below.

GUIDELINES FOR USE
Welcome to the surveyor inspection form. This form has been semi-automated for ease of use.
Please follow below guidelines for optimal practice:
1. SHIP TYPE
This form is ship-type specific. Please select the correct ship type on the homepage before starting a survey. If no ship type is selected, irrelevant questions will be visible, as well as doubles in some areas.
2. FORM FILLING
Each question has a Yes/No/Not applicable/Tested column. One column should be marked for each question.
Yes means that every aspect of the item is satisfactory.
No generates an item on the findings list. If “No” is selected, write your remark in the “Deficiency/comment” section. You are also encouraged to write comments even if the item is satisfactory.
N/A means that the question is not applicable for your vessel.
Where tested, please mark the “Tested” column as well. If not inspected, leave the line blank.
On the bottom of each section you have space to write general comments. Guidelines are provided for your assistance.
3. HOME PAGE BUTTONS
START SURVEY: Push this button after selecting a ship type to have the appropriate inspection form for your survey. This button launches the survey.
PRINT INSPECTION FORMS: The “print” button opens a dialog where you may select a physical printer, or a pdf printer. All info which is filled in the sheet will be printed. You can use this button to print (or pdf) empty forms before start of survey or for manual filling, as well as to print the completed inspection to leave on board.
PRINT CERTIFICATE OF INSPECTION: On your printer of choice (or pdf) this prints the “Cargo ship certificate of inspection” which should be left on board and posted in a visible place. If the Home page is filled out correctly, the vessel and survey information will also be pre-printed on the certificate. If nothing is filled, a blank certificate will be printed.
PRINT AUTOMATIC FINDINGS LIST: This button makes a defect list and prints all items in the inspection sheet where you have filled “no”, as well as their comments.
PRINT HANDWRITTEN FINDINGS LIST: This button generates and prints (or makes a pdf of) a defect list which can be filled by hand. If you have filled out ship info on the front page, it will be printed as well. Otherwise this creates a blank deficiency list.
5. WHAT TO LEAVE ON BOARD
Please leave a copy of the full inspection format and findings list on board upon completion of the survey. Also provide a copy of the inspection certificate, to be posted in a prominent place on board the ship.
6. WHAT TO SEND TO BMRS
