Requests to change a vessel’s name must be submitted on Form 04 by email to the Registry.
Where the Registry is informed of the intention to change a vessel’s name, the owner must obtain written approval for the name change from any existing mortgagees. The owner must forward the mortgagee’s written consent together with the Notice of Name Proposed form to the Registry.
